Incidents involving juvenile rollercoasters

Four separate incidents involving juvenile rollercoasters were reported during the review period, including three derailment events. Investigations identified that limited lateral engagement between wheel assemblies and track systems. This can increase the risk of derailment where wheel wear or alignment issues develop over time. Operators should ensure daily visual inspections are undertaken, with dimensional checks incorporated into both in-service and annual inspection regimes. Further technical guidance is expected to be issued in due course.

The importance of reviewing ride design requirements against operational controls has also been highlighted. Particular attention should be given to height restriction controls, ensuring that operational procedures, measuring equipment, staff training, and enforcement of minimum rider requirements remain consistent with the ride’s design and risk assessment documentation.

Incidents involving rider ejection on thrill rides

Two incidents involving rider ejection were investigated. Findings highlighted issues relating to secondary locking systems, pre-use inspection procedures, functional testing, and the effectiveness of restraint checks carried out during ride operation.

These incidents reinforce the importance of thorough restraint inspections before and during operation, together with adherence to relevant manufacturer guidance and industry safety bulletins.

Recent serious ride incident

A serious incident involving passenger ejection from a ride highlighted the importance of maintenance activities and routine inspection processes. Industry guidance has previously been issued covering safety-critical maintenance requirements and inspection considerations.

Further guidance is planned regarding the selection, use, inspection, and maintenance of retaining clips and similar safety-critical components used on amusement devices. This guidance will be relevant to ride controllers, operators, maintenance personnel, and Inspection Bodies involved in pre-use and annual inspections.

Incorrect selection, installation, or maintenance of such components may result in unsafe.
